Completed applications must be received by 5 pm on Monday May 2, 2022 by personally delivering to the dance studio, emailing to info@balletfg.org or by mailing to Ballet Forest Grove, PO Box 1170, Forest Grove, OR 97116.
- Mailed applications must be received (not post-marked) by the deadline.
- LATE SUBMISSIONS WILL NOT BE ACCEPTED
